While online education can be convenient (such as not needing to drive to campus) it can come with other problems (such as connectivity issues). 

This ungraded survey is intended to gather information about any issues you may face or anticipate as we start our online education course for the semester. Please fill out these answers honestly. I will read them and direct you to campus resources that may be helpful.
